D. Michael Forman, M.D.


Dr. Forman was born in Virginia and grew up in Florida obtaining his undergraduate degree in microbiology from the University of Florida and his medical degree from the University of Miami. Dr. Forman served as a chief resident in the Department of Anesthesiology/ Pain Management during residency at the University of Miami/Jackson Memorial Hospital.

Dr. Forman has been in private practice in South Texas for over 25 years previously serving as Chief of Anesthesia services at Valley Baptist Medical Center- Harlingen where he currently holds staff privileges.

He’s a long-standing member of the American Academy of Pain Medicine, American Society of Interventional Pain Physicians, Texas Pain Society, and the Texas Medical Association. He is also a member of the American Society for Laser Medicine & Surgery and the American Academy of Cosmetic Surgery. He is board certified in Anesthesiology and pain management and a clinical assistant professor at the University of Texas-Rio Grande Valley.

Dr. Forman believes in personally spending time with his patients, educating patients to be the best of his ability to facilitate the management of painful and cosmetic conditions.

In his free time, Dr. Forman enjoys golfing, fishing, and bird hunting. He is an avid Florida gator sports fan. He is married to his wife of 30 years, Tracia. Together, they have three wonderful and accomplished adult children.
